Consent to the processing of personal data for marketing purposes

I consent to the company SQS Vláknová optika a.s., with its registered office at Komenského 304, Nová Paka 50901, Czech Republic, ID number: 60913037 (hereinafter referred to as the "controller"), processing the following personal data in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council:

Name and Surname,
email (hereinafter referred to as "personal data")

The Controller will process the above-mentioned data for marketing purposes, specifically for sending newsletters from the Controller to the person who gives this consent. This consent is given for a period of 5 years. The person providing this consent acknowledges that the Controller will process the above-mentioned personal data for the duration of the consent.
The personal data will be processed solely by the Controller, or by a third party with whom the Controller has a properly concluded data processing agreement.
Consent can be withdrawn at any time, for example, by sending an email or letter to the contact details of SQS Vláknová optika a.s., Komenského 304, Nová Paka 50901, public@sqs-fiber.cz.
SQS Vláknová optika a.s. informs its customers that they have the right to:
1. Obtain confirmation from the Controller whether personal data concerning them is being processed or not, and if it is being processed, they have the right to access such personal data and the following information: the purposes of the processing, categories of personal data concerned, recipients or categories of recipients to whom the personal data have been or will be disclosed, especially recipients in third countries or international organizations, the planned duration for which the personal data will be stored, or if that is not possible to determine, the criteria used to define that period, the existence of the right to request rectification or erasure of personal data concerning the data subject, or restriction of its processing, and the right to object to such processing, and the right to lodge a complaint with a supervisory authority.
2. Request the Controller to rectify inaccurate personal data concerning them without undue delay. Taking into account the purposes of the processing, they have the right to complete incomplete personal data, including by providing a supplementary statement.
3. Request the Controller to erase personal data concerning them without undue delay, and the Controller is obliged to erase personal data without undue delay if one of the following reasons applies: the personal data are no longer necessary for the purposes for which they were collected or otherwise processed, the customer withdraws consent and there is no other legal basis for processing, the personal data have been processed unlawfully, personal data must be erased to comply with a legal obligation under EU law or the law of the member state to which the Controller is subject.
4. Request the Controller to restrict the processing of their personal data if they dispute its accuracy for the period necessary for the Controller to verify this fact, if the processing is unlawful and they request restricted processing instead of erasure, or if the data are no longer necessary for the processing purposes, but they need them for the establishment, exercise, or defense of legal claims.
5. Request the Controller to transfer their personal data to another controller designated by them.
6. Object to the processing of their personal data by the Controller.
7. If there is any doubt about whether the Controller is processing their personal data in accordance with the aforementioned regulations, they may contact both the Controller and the Office for Personal Data Protection.
